Activity Entry Follow-up

When in Activity Entry and you need to remove a student follow-up entry, click the “Clear” button. Do not delete.
If cursor is in the Follow-up area, delete will remove the student and all associated information.
If cursor is not in the Follow-up area, delete will only remove the Activity entry (encounter).  Click here an for example.

IDR Form 108/108B for Child Care / Head Start

Question: There are some five-year-old students at our Pre-School that are not included on the CHIP-AZ generated Form 108 and Form 108B for 2-5 year olds? 

Answer: This is a good question and one we are asked every year.  Our sampling of preschool students is intentionally limited.  We do not collect data on preschool students who are 5 years old before 10/1 of the school year.  There is no need to submit any data on students who are 5 years old before 10/1 of the school unless they are enrolled in kindergarten and included in the kindergarten report.

It is important to note that there is no grace period for the 28-day interval required between MMR and Varicella doses.  If MMR and Varicella are not given on the same day, they must be separated by at least 28 days.

CHIP student immunization record is acceptable hard copy.

“... the Arizona Immunization Program Office will accept the CHIP form as the school record.  We do request/require that the record be printed out and a copy placed in the student's record.”

...We allow the printed CHIP immunization record to be printed and used in place of the ASIR because its format matches the ASIR.  The ASIR, and the CHIP facsimile of the ASIR, need to be signed by the person who reviewed the child’s records and established the child’s immunization status as complete or in need of follow up.  An electronic signature is acceptable...

Occasionally your computer runs out of resources (energy) and does some squirrelly things like not displaying the values in a drop-down list. Usually, it just needs to be restarted.
Close CHIP-AZ.
Select the Windows "Start" button, then "Shut Down" and "Restart".
After your computer is back up, open CHIP-AZ again.
 

CHIP users may find this tool useful to create .pdf and email CHIP reports such as Immunization or Injury Incident reports.

Providers

Who are "Other" and "Transferring" Providers?
We put 'Other' in the program, to cover medications or procedures done by someone else while a student is on a field trip. That person can then sign the original permission form.
'Transferring' is there for schools who use the 'Transfer Data' from school to school to prevent the school nurse at the receiving school from being attached to another nurses records.
You should not be inactivating providers that are still current providers at the school and especially 'Other' or 'Transferring'.
"Other" and "Transferring" Providers are excluded from DSC (formerly MIPS billing) and ASHAR reporting.
 

Zip Code

When entering a student address, the valid zip codes from which you can choose in the drop down list are limited to those which have been set up for the CITY you have entered for the student.

So, if you enter Flagstaff as the city and try to enter 85026, you will get an error. However, if you enter Phoenix as the city and try to enter 85026, you will NOT get an error.

Yes, certain zip codes; like 85027 and 85253 span city lines.

Yes, you can enter a new zip code. Go to CHIP-AZ Utilities tab, Zip Code Addition button.
