Must be reentered each
year. Dear Nurses, IEP's and their contents can change from year to year or be discontinued, so it is necessary to reassess just as you would medications or daily procedure set ups. It is very easy to change year back and print a short list of IEP's and reenter from that if still applicable. -Mary |

Form 108/108B for Child Care / Head Start
Answer: This is
a good question and one we are asked every year. Our sampling of preschool
students is intentionally limited. We do not collect data on preschool
students who are 5 years old before 10/1 of the school year. There is no
need to submit any data on students who are 5 years old before 10/1 of the
school unless they are enrolled in kindergarten and included in the
kindergarten report. |

| Immunization Grace Period |
Arizona Department of
Health Services does now accept immunizations (including MMR) given
within a 4-day grace period, in accordance with the intervals allowed by
the Advisory Committee on Immunization Practices (ACIP). A notice to
this effect is posted on our website at
http://www.azdhs.gov/phs/immun/pdf/4-dayGracePeriodAIPOStatement.pdf.
It is important to note that there is no grace period for the 28-day interval required between MMR and Varicella doses. If MMR and Varicella are not given on the same day, they must be separated by at least 28 days.

CHIP student immunization record is acceptable hard copy. “... the Arizona Immunization Program Office will accept the CHIP form as the school record. We do request/require that the record be printed out and a copy placed in the student's record.”

student address, the valid zip codes from which you can choose in the drop
down list are limited to those which have been set up for the CITY you have
entered for the student.
Yes, you can
enter a new zip code. Go to CHIP-AZ Utilities tab, Zip Code Addition button. |
