your 2007-2008 ASHAR Data Export. Please do it anytime during last week of school.  Be sure to change the Begin and End dates to reflect first and last day of school for students.

Immunization News-
Arizona Department of Health Services does now accept immunizations (including MMR) given within a 4-day grace period, in accordance with the intervals allowed by the Advisory Committee on Immunization Practices (ACIP).  A notice to this effect is posted on our website at http://www.azdhs.gov/phs/immun/pdf/4-dayGracePeriodAIPOStatement.pdf.  

It is important to note that there is no grace period for the 28-day interval required between MMR and Varicella doses.  If MMR and Varicella are not given on the same day, they must be separated by at least 28 days.

  MS Office 2007 - CHIP-AZ will not be compatible with MS Office 2007 until at least 1/2008. Do not plan to install MS Office 2007 before compatibility testing is completed.

CHIP-AZ version 10.0 will be released as soon as possible after we receive IDR changes from ADHS. We have added Influenza, HPV, Meningitis, and  Pneumococcal vaccine tracking to v10.0.

Hearing Reporting Must Be Mailed via US Postal Service.
It is not uploaded with Year End ASHAR Data Export.
