Attention to all School Health Office Personnel:

While extracting some data from your CHIP generated ASHAR End of Year Reports for 2009-2010, it has come to our attention that some schools have very low totals of the codes 280 and 285 being documented.  

• 280 is the coding that should be entered when an RN or LPN sends a student home, and

• 285 is the coding that should be entered when a Health Assistant or other office personnel send a student home. 

• These codes are not automatically created if you send a student home.  

• Please review all your activity entries for the 2010-2011 school year before uploading your final report in May or June for any you may have forgotten to code a 280 or 285. 

• You can change or add codes after activity entry has been checked done.

v13.1 glitch - Immunization Data Reporting - Assessment Form 108-B (0-17 months) and Assessment Form 108-B (18-60 months) show November 15, 2009 at top. Please cross out 2009 and write in 2010.
 

Message from Jennifer Ralston-King from Arizona Immunization Program Office (AIPO):

This year web-based IDRs are available for kindergarten, sixth and tenth grades online at https://app.azdhs.gov/IDR/Forms/NewSchoolImmunizationForm.aspx.

Schools in all counties except Pima are required to submit their IDR's for kindergarten, sixth and tenth grades online this year. We have not made paper IDR's available for use by schools in any counties except Pima. Schools were sent e-mail messages telling them about the new system.

Schools that use CHIP-AZ, and schools that do not use CHIP-AZ, are required to submit their data into our (AIPO) web-based forms.  

• Preschool/childcare reports are accepted on CHIP generated paper report at this time except for Pima County.

• AIPO does accept CHIP generated paper preschool reports.

Message from CHIP-AZ Coordinator – Mary Hallett:

• You can use CHIP generated reports to supply you with the required data for the web-based IDR's. 

• Please do not send paper forms to ADHS for Kg, 6^th or 10^th grade IDR’s.

• Please ignore the mailing address on the CHIP forms for these reports.

• Preschool IDR is the only paper report to be mailed.  Remember to mail both 108 and 108B forms.

No testing has been done or will be done with Access 2010 before start of school year 2010-11.

"I'm not sure how many PowerSchool Districts you service with CHIP.  PowerSchool came out with a new Health piece, I told" (district) "that it was NOT what we needed for AZ and our nurses."  - IT person

After the release of 12.3 version of CHIP it has come to our attention that when printing Color Vision Referral Letters that they will all print in Spanish even if home language is noted as English on Review, Student, information page. 

To print in English, you will need to go to Review, Student, information page and check the box beside 'English Referrals' on that page for each student you want an English referral on.

We apologize for the inconvenience.  In our attempt to fix something else we broke Color Vision referrals.
 

Above does not satisfy the ADHS need. You still need to Mail CHIP IDR or ADHS form to ADHS or Create and submit electronically to ADHS the ADHS spreadsheet.
 

Hi Mary,
"Thanks for your e-mail.  We are accepting CHIP forms along with our own printable IDR forms. The only calls we are making to schools are to follow up on incomplete forms or outdated forms, and to return calls from school personnel who have left messages. While we prefer that schools submit their information electronically, it is not required.  I have double-checked with the temporary employee who is assisting me and her understanding is the same as mine – we will accept the forms as long as they provide the information we need."

Enrollment Cleanup - Since many Student Information Systems do not provide a ’W’ to your school on students that graduate, we have enhanced in v11.1 the utility Graduated Students – Mark W.
