Congratulations!  You have a unique opportunity to participate in the Child Health Indicator Program (CHIP-AZ).  Through a reciprocal agreement between the Arizona Department of Health Services (ADHS) Public Health Prevention Services (PHPS), Office of the Deputy Assistant Director and participating school nurses, nine child wellness indicators (that aren’t tracked by any other health entity, private or public), are being provided to ADHS, PHPS. Through participation in this program, you have the opportunity to contribute to a body of knowledge reflecting the health status of school age children in Arizona.  This aggregate data may be used to determine future health policies and funding for children in Arizona.
