Screening / Health Entry  Height-Weight

CHIP-AZ needs properly formatted entry of the results for Ht-Wt.
CHIP-AZ attempts to remind you and let you know if the format is not correct. 

Remember:

• Enter Height-Weight results such as 60.5-100.5 with the dash (-) included.
• Please do NOT enter a student without a complete result such as 57.6-
• Please do NOT enter a student without a result.
• If you receive the following error when opening Screening / Health, you probably have a mal-formed result entry.
